Recurring Commitment Form

When the page opens click on the “Field Personnel” tab.

Select the name of the field personnel you wish to give to

IMPORTANT – Click on the gray down-arrow in the box that says “Make a Monthly Recurring Gift.”

Click the checkbox beside “Yes, make this a monthly recurring gift.”

Next, fill in the amount, credit card number, and other information required.

Finally, click the “Give Now” button at the bottom of the page.
If you do not want to set up a monthly commitment using your cedit card, then please fill in all the fields in the form below. The form will not submit with any blank fields. Thank you for your generosity.
Thank you for your willingness to participate financially in this ministry. There are several ways to make your first contribution:
By Mail: Send your gift to “Cooperative Baptist Fellowship, P.O. Box 101699, Atlanta, GA 30392.” Make checks payable to “Cooperative Baptist Fellowship.” Be sure to make a notation in the memo line indicating that your gift is to be designated towards the ministry of the appropriate field personnel. Include a note with your address, phone/email, and whether this is a monthly or occasional gift.
